Semaglutide for MASH: What ESSENCE Shows
ESSENCE improved liver-biopsy outcomes in MASH with F2–F3 fibrosis. What accelerated approval means and which longer-term benefits remain unproven.
Semaglutide improved liver-biopsy outcomes in adults with MASH and moderate to advanced fibrosis in ESSENCE. Wegovy subsequently received accelerated approval for a defined noncirrhotic MASH population. These findings are more specific than “reverses fatty liver”: they do not yet establish a reduction in liver failure, transplantation or death. 1 2
MASH is not a synonym for every fatty-liver finding
Metabolic dysfunction-associated steatohepatitis, or MASH, is the disease named in the trial and indication. The August 2025 Wegovy label specifies adults with noncirrhotic MASH and moderate to advanced fibrosis, consistent with stages F2–F3. That wording does not cover every person with fat seen on a liver scan, and it does not include cirrhosis under this indication. 2
This boundary is central to the decision. Someone may know they have “fatty liver” without knowing whether they have the inflammatory disease or fibrosis stage studied. A weight-loss consultation alone cannot establish that match. The treating clinician should explain the diagnosis, how disease severity was assessed and what the proposed treatment is intended to accomplish.
The published result was an interim analysis
ESSENCE enrolled 1,197 participants in a randomized, double-blind, placebo-controlled phase 3 trial. Participants were assigned in a two-to-one ratio to weekly injected semaglutide 2.4 mg or placebo. The published 72-week analysis involved the first 800 participants, while the study's planned duration extended to 240 weeks. 1
Those are different denominators and time periods. Describing the paper as “1,197 people followed for 240 weeks with proven liver benefit” would combine the overall enrollment and planned duration with an earlier analysis. The result is substantial, but it should be reported at the point actually measured.
The two primary biopsy outcomes required improvement in one feature without worsening the other. One was resolution of steatohepatitis without worsening fibrosis. The other was improvement in fibrosis without worsening steatohepatitis. That is more informative than an isolated change in body weight or a liver-enzyme result, but still different from observing fewer transplants or deaths.
The 72-week ESSENCE outcomes
| Outcome | Semaglutide | Placebo |
|---|---|---|
| MASH resolution without worse fibrosis | 62.9% | 34.3% |
| Fibrosis improvement without worse MASH | 36.8% | 22.4% |
| Both resolution and fibrosis improvement | 32.7% | 16.1% |
The estimated difference for the first primary outcome was 28.7 percentage points, with a 95% confidence interval of 21.1–36.2. For fibrosis improvement it was 14.4 percentage points, with a confidence interval of 7.5–21.3. The published estimates account for the study's analysis; they should not be confused with a claim that every treated person improved. 1
What accelerated approval does and does not mean
The FDA label states that the MASH indication received accelerated approval based on improvement in MASH and fibrosis. Continued approval may depend on verification and description of clinical benefit in a confirmatory trial. This is a real approval with an explicit evidence condition, not an assertion that every long-term outcome is already settled. 2
A biopsy outcome can be important without being the same as how a patient ultimately feels or survives. The key question is whether improvements in the measured disease features translate into fewer serious clinical events over longer follow-up. Our explanation of surrogate endpoints and patient outcomes describes why that distinction matters across peptide research.
Avoid two opposite mistakes. Calling the evidence meaningless because it uses biopsy endpoints dismisses a randomized improvement in liver disease. Calling it proof of a cure ignores the outcomes not yet established by this interim report. The accurate conclusion sits between those extremes: meaningful histologic improvement in the studied population, with longer-term clinical benefit still requiring confirmation.
Weight loss and liver improvement are different outcomes
The trial reported mean weight changes of minus 10.5% with semaglutide and minus 2.0% with placebo at the interim assessment. Those weight results are not the percentages of participants whose MASH resolved. Nor do they prove that matching a particular amount of weight loss guarantees the same biopsy response. 1
This distinction is useful when comparing a clinic advertisement with the paper. An advertisement may discuss weight loss, liver health and metabolic benefit in the same paragraph. Each claim still needs its own endpoint and population. A general weight-management study cannot automatically establish the MASH result, and the MASH trial cannot establish the effectiveness of a different compounded product.
The Noom Med review examines self-reported weight results, while the WeightWatchers Clinic review unpacks an observational engagement comparison. These are examples of weight-management program evidence, not liver-treatment results: neither type of outcome can substitute for the biopsy measurements in ESSENCE.
The trial tested a specified injected medicine. It did not compare oral supplements, experimental peptide combinations or every product containing semaglutide. The compound-versus-trial-drug distinction remains relevant even when the active ingredient has persuasive evidence.
Tolerability and follow-up still matter
Gastrointestinal adverse events were more common with semaglutide in ESSENCE. A liver indication does not eliminate the need to assess contraindications, other medicines or symptoms during treatment. The study does not supply an individual dosing plan, and its treatment schedule should not be used to adjust a prescription without the prescriber. 1
Before starting, ask how liver disease will be followed, who will coordinate metabolic and liver care, and what improvement would count as a meaningful response. Also ask how persistent side effects will be handled. A plan that discusses only the number on a scale leaves an important part of the MASH treatment goal unexplained.
A later normal blood test or lower body weight should not be treated as proof that all fibrosis has disappeared. Different measurements answer different questions. The clinician should explain which measurements are being used and what they can reasonably establish rather than promise that any single result means cure.
What a useful claim should say
A responsible summary names noncirrhotic MASH with F2–F3 fibrosis, the 72-week biopsy outcomes, and the accelerated-approval condition. It should also make clear that the study was funded by Novo Nordisk and that the published analysis did not settle long-term liver-failure or survival outcomes. 1 2
That wording gives patients a concrete basis for a conversation. It supports asking whether their disease matches the indication and whether the proposed medicine matches the evidence. It does not support self-diagnosing MASH from an online symptom list or choosing a product solely because a seller uses the phrase “liver health.”
Frequently asked questions
Is Wegovy approved for all fatty liver disease?
The MASH indication specifies adults with noncirrhotic disease and moderate to advanced fibrosis consistent with F2–F3. It is not a blanket indication for every fatty-liver finding.
Did ESSENCE prove fewer liver transplants or deaths?
The published 72-week interim analysis established biopsy improvements. It did not establish those longer-term clinical outcomes.
Does accelerated approval mean the medicine is unapproved?
No. It is an approval based on specified evidence, with continued approval potentially dependent on confirmation of clinical benefit.
